A Brief Overview Perform and assist in various pharmacy assignments, under the supervision of a licensed pharmacist, and other duties not requiring the review of a pharmacist. The majority of tasks and responsibilities are related to preparing, dispensing, and distributing medications, including, but not limited to, compounded non-sterile products (CNSPs), patient specific and non-specific non-sterile medication doses, and prepackaged or unit-dose products containing oral, injectable, or other medications.

What you will do 1. Responsible for accurately preparing patient-specific and non-patient specific doses by ensuring that medications are identified, retrieved, drawn-up, labeled, packaged and stored according to the CHOP Pharmacy Standard Operating Procedures. Responsible for completing required 4 Quadrant Training Programs The correct medications are prepared for the correct patient, in the correct amount, at the appropriate time Medication concentrations and volumes are properly confirmed to match the order details on the patient label or patient pick-report Ensure all patient doses are verified by a pharmacist prior to dispensing - prepared doses are accurately identified, retrieved, drawn-up, labeled, packaged and stored. Doses are prepared and distributed within cycle time to ensure that the turnaround time standards are met Prepare accurately and safely Compounded Non-Sterile Products (CNSPs) in accordance with USP795 and CHOP Pharmacy Standard Operating Procedures (SOP). 2. Responsible for accurately delivering patient-specific medications by ensuring that medications are delivered to the designated medication storage location in the proper patient care unit according to the CHOP Pharmacy Standard Operating Procedures (SOP). Responsible for completing required 4 Quadrant Training Programs 3. Responsible for proper handling and accurate oversight (including, but not limited to, handling, delivery, return, inventory count, expiration monitoring) of floor-stock medications to designated automated dispensing cabinetry according to the CHOP Pharmacy Standard Operating Procedures (SOP). Responsible for completing required 4 Quadrant Training Programs 4. Responsible for proper handling and accurate oversight (including, but not limited to, handling, delivery, return, inventory count, expiration monitoring) of medications in all non-automated distribution systems (i.e. medication trays, narcotic packs, transport bags or kits, etc) according to the CHOP Pharmacy Standard Operating Procedures (SOP). Responsible for completing required 4 Quadrant Training Programs 5. Responsible for compliance to all federal, state, and department standards and regulations for non-sterile product compounding- USP795, USP800, CMS Conditions of Participation, FDA Compounding Guidance and JCAHO standards 6. Responsible for accurately repack ging medications into unit-dose packages using pharmacy repackaging hardware according to the CHOP Pharmacy Standard Operating Procedures (SOP). 7. Responsible for accurately performing Inspection process according to the CHOP Pharmacy Standard Operating Procedures (SOP). 8. Responsible for ensuring that all documentation and recordkeeping requirements set forth by CHOP Pharmacy are met, including but not limited to: PO Preparation logs CNSP Preparation logs Anesthesia Controlled Substance Records Prepack Controlled Substance Records Departmental Health Status Boards 9. Compliant to all mandates set forth by CHOP Pharmacy and by CHOP Human Resources 10. Compliant to all dress code and PPE maintenance and associated CHOP Pharmacy SOPs 11. Inventory Control Kanban Inventory system is adhered to 100% of time. Maintains and organizes inventory supply in a manner which minimizes inventory storage cost, wastage, and stock-outs. Ensures inventory is rotated properly, stored in appropriate conditions and location with 100% accuracy. Complete monthly medication expiration dating assignments in a timely manner. Identify and reorder expired medications inventory and ensures that stock is stored in accordance with manufacturer requirements. Assures the proper replacement of prescription drugs to stock upon requisitions from various departments of the hospital either manually or through the Pyxis System. 12. Ensures the secure storage of narcotics and other controlled substances. Document every transaction completely and accurately. No generation of discrepancies in the C2 Safe, Pyxis Medstation, or Inventory Logs. Ensure the destruction of returned controlled substance continuous medication infusions. Ensures secure storage of narcotics and other controlled substances on a regular basis. Maintains inventory records on narcotics and other forms of drugs as required by law. Performs inventory counts of controlled substances on safes as per policy and procedures. 13. Achieve patient safety and satisfaction through the use of LEAN principles: 5S Elimination of Waste JIT Stand Work Quality Promote and Comply with Standardized Procedures. Promote and Comply with Standard Work. Identifiy waste in ownership process and recommends solutions for implementation. Correct abnormal conditions. 14. Participates in hospital and departmental Continuous Quality Improvement (CQI) activities. Utilizes the Lean Principals of to analyze, and resolve problems in a systematic manner. All improvement must be made in accordance with the scientific method Participation to departmental RPI Work OT to allow coworkers to participate in department RPI Education Qualifications High School Diploma / GED - Required Technical Diploma Pharmacy Technician - Preferred Associate's Degree - Preferred Bachelor's Degree - Preferred

Experience Qualifications Minimum one (1) year of recent retail pharmacy experience - Preferred

Skills and Abilities Basic computer skills including use of Windows operating system and word processors. Calculation skills
