Supportive Services for Veteran Families

$42,500/year

  

SCHEDULE:  Full-time, determined on a case by case basis

 

GENERAL STATEMENT OF DUTIES: Provides case management, care coordination, health education, interdisciplinary collaboration, consultation to veteran households, and supervision to SOARS specialist staff. 

 

ESSENTIAL RESPONSIBILITIES:

  1. Provides services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.

  2. Conducts assessments to understand the Veteran's situation, potential barriers to care, causes, and impact of such barriers on the Veteran's ability to access and maintain health care services.

  3. Works closely with the Veteran's primary care provider and members of the Veteran's assigned multidisciplinary treatment team (ie. medical, nursing, admin specialists, case management personnel) to provide timely, appropriate, Veteran centered care equitably.

  4. Acts as liaison between the SSVF grantee and the VA or community medical clinic.

  5. Serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to needs and desires of the Veteran.

  6. Works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.

  7. Works closely with Veterans to assist them in communicating their preferences in care and personal health-related goals.

  8. Assists Veterans in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team.

  9. Reiterates provider recommendations using clear language to support the Veteran and family members or caregivers.

  10. Documents and records all significant interventions, diagnostic action steps, progress reports, and utilization of referral services.

  11. Evaluates the effectiveness of resources and referrals provided and makes appropriate modifications to ensure the provision of high-quality care and interventions.

  12. Assists in development of policy and procedures using knowledge gained from research or best practices.

  13. Collects and provides required data for the HMIS database, as applicable. 

  14. Oversees SOARS/Income Specialist staff.

  15. Actively participates in regular SSVF Team meetings and other coordination meetings for the purpose of consistency, monitoring and effectiveness.

  16. Applies Motivational Interviewing best practice models in service delivery approach.

  17. Provides crises intervention when necessary.

  18. Participates in community-wide events promoting services for veterans.

  19. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.

  20. Other duties as assigned.

Requirements

QUALIFICATIONS:

  1. Demonstrated organizational, interpersonal, and communication skills

  2. Possesses excellent judgement 

  3. Valid Ohio Driver's License and ability to meet agency requirements for driving insurability if applicable; reliable transportation

  4. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).  

     

MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS:  Bachelor's level social worker
