Shifts: Full time, Monday-Friday 8:30am-5:30pm

Job Responsibilities:

· Triaging patients in person and over the phone

· Performing clinical questionnaire assessments

· Patient education

Skills & Qualifications:

· 6+ months of experience as an LPN in an outpatient setting

· Active LPN license in the state of NY
