Pre-Release Case Manager Overview: We are seeking a passionate Pre-Release Case Manager to support individuals on their re-entry journey from prison. You will help participants develop life goals, build essential skills, and connect them with our broader care team, ensuring a successful transition.

Key Responsibilities:

  • Conduct 1:1 sessions to build trusting relationships and understand participant needs.

  • Deliver curriculum with fidelity and quality.

  • Coordinate applications for Medicaid, food stamps, and other benefits.

  • Prepare logistics for obtaining necessary documents post-release.

  • Use Motivational Interviewing to keep participants engaged.

  • Document sessions and flag participant needs in the electronic record system.

  • Collaborate with a multidisciplinary team to ensure consistent and aligned support.

Skills and Qualifications:

  • Bachelor's degree in social work, psychology, counseling, criminal justice, sociology, or a related field.

  • 1-3 years of experience in case management and documentation.

  • Ability to resolve conflicts, set boundaries, and work with a diverse team.

  • Flexibility to work within correctional facilities.

  • Effective communication skills with stakeholders.

  • Ability to work independently with professionalism, integrity, and compassion.

  • Driver's license and ability to pass DOC security clearance.

  • CPR/Adult First Aid certification preferred.

Pay Details: $55,000.00 to $65,000.00 per year
