They handle a range of clinical responsibilities that demand a combination of basic and advanced nursing expertise, involving both hands-on patient care and supportive tasks. The primary care LPN plays a crucial role in promoting preventive care and addressing any health concerns to enhance the quality of life for patients and their families. Essential Responsibilities Provides direct patient care support in a variety of settings for primary care patients utilizing primary and palliative care principles, trauma informed care and promotes patient well-being and empowerment. Coordinates, schedules, and conducts annual wellness visits with patients, families, and caregivers. Perform medication reconciliation through patient interview, review of medical records, and consulting with other healthcare providers involved in the patient's care. Collaborates with providers to resolve discrepancies or address any care gaps and recommended preventative measures such as mammograms, colonoscopies, lab tests, etc. Engages patient and family in advance care planning including discussions about goals of care and answering general questions about advanced directives and assisting patients with documentation if requested. Educate patients about their health conditions, treatment options, and self-management strategies to promote active participation in their care. Coordinates, schedules and accurately, performs point of care testing; communicates findings to the healthcare team for further evaluation and treatment. Communicates test results with patients when deemed appropriate by the provider or payer. Assesses vaccination needs, discusses vaccination options, obtains informed consent, prepares and administers vaccines, monitors for adverse reactions and provides post vaccine education and documentation. Recognizes signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing and collaborates with patient's provider when identified. Works closely with members of the primary care interdisciplinary team or payor staff to ensure coordinated and comprehensive care and optimal scheduling of patient visits. Collaborate with the healthcare team to develop a responsive plan of action to address crises effectively and ensure patient safety. Practice within the scope of license, individual competencies, certification, or registration, where applicable, and as required by law and regulation. Provides timely and effective patient documentation including data collection. Identify and address organizational, cultural, and other barriers to best practice strategies. Assists in mentoring newly hired staff; may provide coaching and training. Provides back up coverage for primary care nurses, within LPN scope of practice.
