HARBORVIEW’S MEDICATION ASSISTED TREATMENT (MAT) PROGRAM has an outstanding opportunity for a PROGRAM COORDINATOR WORK SCHEDULE
100%FTE, 40 hours per week
Day Shift POSITION HIGHLIGHTS
Coordinate the delivery of medical care, mental health, recovery, and other social services for patients requiring medication-assisted treatment for opioiduse disorder and access to health care
Be a part of a dedicated multidisciplinary team DEPARTMENT DESCRIPTION Harborview Addictions Program (HAP) clinic and Harborview Mental Health Services (HMHS) clinics will coordinate medication assisted treatment along with collaborative, collocated, comprehensive mental health and substance use disorder treatment Services. HAP and HMHS clinics have a longstanding focus on dual diagnosis patients who are patients with chronic serious mental health issues and chemical and/or alcohol use/abuse issues. Many of these patients are also experiencing homelessness, have untreated mental and physical health conditions, and require legal and other case management services due to their opioid use disorder, highlighting the need for integration of MAT with comprehensive services
JOB RESPONSIBILITIES
Handle intake of all referral requests, ensuring complete information is received and entered into the computer system
Provide telephone screening of patient medical problems and severity of condition. Triage patient telephone calls to clinical staff; initiate emergencyprocedure when necessary. Coordinates incoming and outgoing referrals
Conduct outreach through email to HMC clinics, providers to offer linkage services to patients identified as targeted client base
Coordinate with personnel from Public Health – Seattle & King County, other treatment programs and area clinic managers and medical providers to provideinformation on MAT
Coordinate with case managers and inpatient medical providers to assess need for linkage to MAT care among persons hospitalized in HMC or in aftercare
Develop and maintain procedural standards for outreach efforts
Provide messaging and instructions to patients about MAT, about screening, and treatment
Implement a screening questionnaire to each prospective patient to assess eligibility and fit for the program
Interview patients and review anticipated needs upon admission to the program and discharge, including personal escort, transportation, housing,support care, equipment needs, transfer of records, and appointments with physicians and care managers
Ensure complete and accurate registration of patients to receive MAT services, including patient demographic and insurance referral information arein the computer system
Ensure a complete medical record is present at the time of patient visit, admission, and/or procedure
Act as resource to the team for payor issues including insurance knowledge for Medicare, Medicaid and commercial plans
Facilitate prior authorization with insurers for payment of treatment
Coordinate with the case managers, financial counseling and the contact center for verification of benefits and authorization for outpatient visits
Refer patients to Financial Counselors
Perform office tasks as needed including distribution of mail, creation of clinic-specific patient packets, and processing loose medical records papers REQUIREMENTS
High school graduation or equivalent AND two years of experience in the program specialty
