COUNTY OF GREENE Social Worker II - CAP/DA in Snow Hill, North Carolina
The purpose of this position is to provide long term case management of clients and determine a plan of care designed to provide an alternative to institutional placement through community living if non-institutional community services are feasible, preferred by the client and available at a lower cost. Long term case management works with a designated target population for the purpose of home maintenance. The Social Worker must identify mechanisms for referral, screening, development of service plans, case management and reassessment of the target population identified for services in the county, plus evaluation of the program in the consultation with a multidisciplinary team. The Social Worker must identify and coordinate the use of various funding sources for screening, case management and provision of in-home and community based services.
