The Clinical Liaison provides hospice expertise to community settings and potential clients and their families. This person will perform hospice consults, and coordinates with hospice team to meet immediate needs of client, including medication and DME. The Clinical Liaison builds positive relationships with key referral sources and establishes client's eligibility for care provided. Qualifications * RN required * College degree in nursing, RN preferred. * Minimum 2 years' experience in hospice or palliative care. Comfortable facilitating end-of-life and goals of care conversations with clients and families. * Strong professional network. * Ability to perform presentation and public speaking. * Proficient with technology and technological applications: Microsoft Office, Smart Phone, Calendaring, Wi-Fi, EMR, Internet Browsing, Printer Knowledge, Skills and Abilities * Teams: skilled at collaboration, works effectively as part of a care team * Customer-focus demonstrates empathy, a desire to serve, and reliability. * Relationship Builder (phone and in-person), Approachable, Outgoing, Friendly * Influencing: confident and comfortable promoting proactive solutions * Personal resiliency: emotional/intellectual flexibility, ability to remain calm, stay focused and maintain objectivity in stressful situations. * Self-directed: demonstrates initiative and works independently * Productivity: skills in prioritization, planning, organizing and time management * Verification of COVID vaccine status or receiving an approved exemption.
