We group providers into care teams to enhance patient care and improve access. These care teams are broken down based on the clinics in which providers practice. Position Summary Build your primary care practice at an independent organization committed to integrity, compassion, accountability, high quality and innovation. Care for individuals and families throughout central Minnesota by providing preventative screenings, address immediate health needs and assist in managing chronic conditions. Collaborate with specialists and support services to empower health and wellbeing together for your patients. Position Type This position will be housed at our Browerville and Eagle Bend clinics, with operating hours from 8:30am-4:30pm. Orientation will be held at our Staples location for the first six months of employment. Qualifications Qualifications Licensed Nurse Practitioner or Physician Assistant in the State of Minnesota Ideal Candidate Promotes trust with their patients by taking time to build relationships Collaborates with fellow family medicine providers to provide the best patient care Refers to specialist providers when appropriate Manages clinic visits in a timely manner without rushing patients Addresses patient needs with compassion and integrity Benefits Lakewood Health System is growing and looking for great people to join our mission of providing personalized, quality healthcare for a lifetime.
