Pay & Benefits: The pay range for this role is generally $52,000-$57,000 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

CHASI’s HIV prevention activities include but are not limited to 1) in-person outreach efforts to engage persons at high risk for HIV infection; 2) education of high-risk community members about behavioral practices and medical interventions that can decrease acquisition of HIV; 3) Intake-Assessments and Linkages/Facilitation to pre/post exposure prophylaxis (PrEP/PEP) provider and/or social supportive services. The activities listed above are embedded within comprehensive HIV prevention services provided by CHASI, and may be coordinated with the teams of, and/or conducted within, CHASI’s Mobile Health Units, storefront offices, and The Pride Center of Staten Island, requiring collaboration and coordination with the management and staff of these service sites.

The HIV Prevention Coordinator works with high-risk HIV negative clients who are also substance users, including clients actively enrolled in CHASI’s syringe exchange program (SEP) and 24-hour Next Step Resource Center. The Coordinator works with clients to provide education, prevention materials, and linkage to direct services to high-risk clients to increase their awareness of PEP and PrEP. This position will supervise the Outreach and Education Specialist and stipend Peer Workers.

This is a full-time position that generally works Monday- Friday 9am-5pm and requires a flexible schedule such that some of the job tasks occasionally, with notice, will be conducted during evening hours and on weekends.

DUTIES & RESPONSIBILITIES:

  • Participate in community outreach events to foster relationships with potential clients.

  • Conduct risk behavior screenings to identify persons at high risk of HIV infection.

  • Enroll clients deemed eligible into the prevention program, develop a service plan, safety plans and other documentation to record all services provided.

  • Navigate the client to execute their service plans, including make active referrals, accompany clients to various appointments, provide coaching to clients, deliver weekly or monthly education encounters, plan out logistics for plan adherence reminders, assist with transportation to get to and from appointments.

  • Educate and counsel high risk persons on combination prevention strategies according to client presentation and needs, i.e. correct and consistent condom use, the importance of frequent HIV testing and screens for other sexually transmitted infections, and medical interventions intended to assist efforts at staying HIV-negative.

  • Provide education about and linkage to PrEP and PEP. Following referral to PrEP or PEP, provide follow up with clients provided such referrals.

  • Contact clients via phone, text, and email should they fail to attend appointments or stray from documented service planning.

  • Directly supervise an Outreach & Education Specialist and stipend Peer Outreach Workers.

  • Conduct Peer trainings & check-ins.

  • Prepare Peer-related Stipend submissions to the finance department

  • Work collaboratively with internal departments and external partners.

  • Collect and submit data into AWARDS daily. Document accurate data on persons contacted and engaged via both virtual and in-person outreach. Participate in documentation quality assurance and reconciliation tasks.

  • Keep up to date weekly deliverable tracker.

  • Participate in internal and external meetings and training as assigned.

  • Align all activities with CHASI’s mission, core values, and culture.

  • Other duties as assigned.

Requirements

QUALIFICATIONS:

  • At least 3 years of related job experience required. Bachelor’s degree may be considered in lieu of some years of experience.

  • High school diploma or high school equivalency required.

  • At least 1 year of experience supervising staff and/or program management experience and/or having opportunities to demonstrate this skillset. Must be able to lead teams made up of staff from various departments in a positive way for optimal development and growth.

  • Experience in community outreach events

  • Willing to complete the Recovery Coach Certification within 6 months of hire.

  • Ability to engage and serve Individuals affected by substance use as well as individuals engaging in opioid use, as prioritized by this program.

  • Fluency in Spanish highly preferred.

  • Must have basic knowledge regarding HIV, HCV, STI, PEP/PrEP and Harm Reduction methods.

  • Preferred experience in PrEP AP, Gilead, and applying for copay cards.

  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).

  • Ability to work independently and as a team; be team player with an ability to work with diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment that motivates others to perform at their best.

  • Must possess the ability to interact with all levels of external and internal partners with a high level of professionalism and tact in representing CHASI.

  • Strong verbal and written communication skills, including capability to listen and follow instructions; able to give, provide direction and follow up on task; high attention to detail and accuracy. Be highly organized and able to multitask.
