Loretto Management Corporation Physical Therapist - PACE of CNY in Syracuse, New York

Overview

Under the general supervision of the Director of Rehab, the Physical Therapist evaluates for physical therapy services, establish a treatment plan, provide appropriate treatment; provides necessary consultation and teaching to participants/family/caregivers and interdisciplinary team.

Responsibilities

Job duties specific to this position:

  • Instructs paraprofessional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, home exercise program, etc.

  • Educates participants in fall prevention techniques and home exercise programs; incorporates the teaching of PT skills and techniques necessary to promote participant independence.

  • Educates family and caregivers in appropriate positioning, exercise or mobility techniques.

  • Supervises paraprofessional staff in the performance of their duties as they relate to Physical Therapy.

  • Responsible for the completion of all participant documentation in accordance with program policy, including routine progress notes, communication to physicians, nursing and participants/families and evaluation/discharge summaries.

  • Maintains consistent written and verbal communication regarding a participant’s care to other program providers/contracted providers.

  • Has oversight of maintenance exercise programs as needed.

  • As member of interdisciplinary team performs pre-enrollment assessments of potential enrollees as needed; performs initial evaluation on new participants; participates in periodic reassessments when participant is receiving or in need of physical therapy services; additionally completes annual assessments.

  • Responsible for reviewing and interpreting all referrals from providers and discussing any questions or concerns regarding participant’s care.

  • Participates in the development and revision of the participant’s Plan of Care as a member of the Interdisciplinary Team; develops maintenance or restorative treatment plans as appropriate.

  • Provides ongoing re-evaluation of the treatment programs.

  • Participates in home assessments, as indicated, to enhance the multidisciplinary goals toward participants’ independence and safety at home.

  • Establishes goals that recognize the multidisciplinary rehabilitation needs of the participant.

  • Makes recommendations for equipment; transportation; makes community services referrals for participant and rehabilitative support for family.

  • Participates in morning meetings, family meetings, reassessment meetings nursing home care planning meetings, and clinical case conference as appropriate.

  • Makes recommendations that improve program operations and further develops quality physical therapy services at PACE; participates in the Quality Assurance program and serves on the Quality Assurance Committee, as requested; reporting to team as needed, coordinates fall discussions at morning meeting; participates in task forces as requested.

  • Participates in the orientation of new staff to the operation of the Physical Therapy Department.

  • Participates in student programs established with physical therapists or physical therapist assistant schools, in the effort to promote the entry of qualified individuals into the profession.

  • Promotes professional growth, the profession of physical therapy and the program, when possible, by participation in profession associations, educational conferences, volunteer activities, etc.

  • Demonstrates adherence to all compliance policies and procedures. Is responsible for promoting and fostering compliance in the workplace.

  • Ensures the highest quality of care by performing responsibilities according to the highest professional standards.

  • Demonstrates knowledge of PACE/D&TC Emergency Preparedness Plan and can verbalize their role during an event.

Qualifications

  • Degree in Physical Therapy from an accredited program in school of physical therapy, Doctorate preferred.

  • NYS license to practice as a Physical Therapist.

  • Minimum of one year’s experience in geriatrics required.

  • Demonstrates clinical competencies as a Physical Therapist.

  • Strong interpersonal skills; excellent written and verbal communication skills.

  • Valid driver’s license and a reliable vehicle.

  • Proficient in MS Office and able to learn new software rapidly.

An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
