Under the guidance and direction of the Home Care Manager, theHome Care Case Manager RNwill participant with planning, organizing, implementing and reviewing of home care services, as well as providing direct physical nursing care to the patient. The Home Care Case Manager RN is responsible for providing the highest quality of care in their PACE participants’ homes and ensuring safety at all times according to accepted nursing practice standards, which includes health teaching, delegation to other nursing and paraprofessional staff, and acting as a member and a liaison between the PACE CNY Interdisciplinary Team and the participant and family/caregivers.

Responsibilities
The RN Case Manageroversees a number of PACE participants and is responsible for all their medical care; ensuring the medications are accurate and medication plan is right for the LPN to follow. The RN Home Care Case Manager develops a plan of care that the HHA will follow for safety and quality care (meal prep needs, dressing assist, bathing, transferring, ambulating, medication management, etc.) They are responsible for oversight on all of these plans and making sure the care is provided. In addition, the RN Case Manager also attends meetings and completes their own assessments as needed. There are some acute skill needs (IV’s, PICCS, Foleys, etc.).
Additionally, The RN Case Manager acts as a primary support to the Physician, Nurse Practitioner, Director of Clinical Services and Home Care Manager. Provides quality care through assessment, development, implementation and evaluation of individualized participant care plans.
Additional Responsibilities :
Provides quality care through assessment, development, implementation and evaluation of individualized participant care plans.
Teaches and counsels participants and caregivers as needed regarding maintenance of health and prevention of illness or complications.
Delegates tasks appropriately and supervises, instructs and guides nursing and paraprofessional staff in providing high quality nursing services.
Contributes as a member of the PACE Interdisciplinary Team. Acts as a liaison between the participant, the family/care givers, the home care supervisor and the PACE Interdisciplinary Team.
Provides leadership among co-workers.
Documents in an appropriate, thorough and timely manner and in accordance with Loretto Geriatric Center Licensed Home Care Agency’s medical records policies and procedures.
Qualifications
Degree from an accredited school of nursing.
Current NYS license and registration as a registered professional nurse required.
Minimum of one-year recent medical surgical experience as a RN in an acute care setting.
Must possess a valid driver’s license without restrictions and a reliable, insured vehicle.
