Job Summary and Qualifications
The role of an Advanced Practice Provider (APP) is essential in hospital-based clinics as they contribute to delivering top-quality, patient-focused care. They work alone while maintaining an interprofessional approach with physicians for all aspects of patient care. APPs comprise of Physician Assistants, Nurse Practitioners, Certified Registered Nurse Anesthetists, Clinical Nurse Specialists, and Certified Nurse Midwives. They offer medical-level care and perform surgical tasks within their state's scope of practice, along with clinical privileges granted by the hospital. The APP aligns with the organization's vision, mission, and values to support an exceptional patient experience and optimal clinical outcomes.
What will you do in this role:
Uses advanced clinical judgment and clinical reasoning to diagnose the patient. Synthesizes relevant data to develop a patient-centered, evidence-based treatment plan of care. Manages care across the health continuum, and records progress notes.
Orders and evaluates diagnostic tests, procedures, and therapeutic modalities and interventions.
Orders non-scheduled medications. Orders scheduled medications (II-V) as permitted within state and federal regulations per relevant specialty and certification.
Performs procedures in accordance with defined qualifications and competencies based on licensure, related state practice act, and approved hospital privileges.
Educates patients/families/caregivers about the patient's medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.
Educates patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management.
Serves as a resource for hospital/clinic employees in the provision of patient care by providing professional advice and guidance, including rapid response or code team response, as appropriate.
