Health First HFMG Clinical Dietitian II - Hematology Oncology -Melbourne in Titusville, Florida

POSITION SUMMARY:

To be fully engaged in providing Quality/No Harm, Customer Service and Stewardship by coordinating all phases of nutrition care including nutrition assessment, nutrition diagnosis, interventions, monitoring evaluating outcomes and counseling clients on a variety of settings. Performs duties in support of and in compliance with performance improvement plans, JC and other licensing, accreditation and regulatory agencies.

PRIMARY ACCOUNTABILITIES:

Quality/No Harm:

  • Recommends or makes appropriate changes to the nutrition care provided to patients/clients.
  • Prioritizes the nutrition diagnosis(es) based on problem severity, safety, client needs, likelihood that nutrition interventions will influence the problem and the client’s perception of importance using specialty level clinical judgment skills.
  • Adjusts guidelines/protocols based on the individual and progress of interventions.
  • Anticipates potential complications to nutrition interventions taking into account potential pharmacotherapy, nutrition and medication delivery systems and develops plans to address them.
  • Generates appropriate nutrition care plans to meet unique needs of patients with complex conditions.
  • Obtains diagnostic test/procedure results to evaluate therapy compliance and/or to identify unanticipated findings and initiates appropriate referrals to the interdisciplinary team members.
  • Uses appropriate data collection tools, assesses and analyzes outcomes data; develops plans to improve outcomes for the system.
  • Monitors and evaluates outcomes related to the nutrition diagnosis and client goals to determine the degree to which progress is being made and nutrition goals are being met.
  • Performs nutrition assessments based on a Nutrition Diagnosis through a systematic process of obtaining, verifying, and interpreting data to make decisions about the nature and cause of nutrition related-problems.
  • Supports superior quality through adherence to the I-CARE commitments.
  • Identifies and implements appropriate actions designed with the intent of changing nutrition-related behavior, risk factors, environmental condition, or other aspect of health status of clients.
  • Evaluates the quality and effectiveness of practice and revises practices as needed to assure continuous improvement of services. May include work with organizational committees or chart audits.
  • Participates in nutrition care of patients in area of certification/ specialty.

Stewardship:

  • Develops processes and guidelines in accordance with best practice to initiate or discontinue appropriate nutrition support and/or oral supplements.
  • Audits and evaluates data to collect, document, analyze and share outcomes for strategic planning.
  • Serve in a leadership role to improve interventions and outcomes in area of specialty.
  • Coordinates effective delivery of nutrition programs; understand revenue streams in area of specialty and insurance reimbursement trends.
  • Participates in the evaluation and selection of new products and equipment related to area of specialty.
  • Proactively recognizes needs, and outcomes of different approaches to achieve desired outcomes.
  • Compares performance of the organization to goals and expected outcomes and develops plans for improvement in area of specialty.
  • Interprets public health trends and applies this knowledge to professional practice and the organization to proactively develop programs and standards in area of specialty.

Customer Experience:

  • Educates patient/clients, family members and health care team regarding nutritional needs, recommending diet, prevention and/or and pertinent issues related to the client’s nutritional care.
  • Provides diet education to Health First associates as requested.
  • Understand theoretical strategies related to behavior change and supports patient readiness to change.
  • Incorporates knowledge of cultural diversity when integrating research in specific patient care.
  • Assess system barriers and facilitate adoption of processes to overcome them.
  • Designs and evaluates marketing strategies for Nutritional Services in area of expertise.

QUALIFICATIONS REQUIRED:

  • Registered Dietitian (RD) by the Commission on Dietetic Registration;
  • Licensed Dietitian/Nutritionist (LDN) by the state of Florida
  • Commission on Dietetic Registration Board Certified Specialist or Clinical Nutrition Support Clinician or CORTRAK certification or Certified Diabetes Educator.
