Housing Resource Community Coordinator:

Department: Short-Term Programs

Reports to: Short-Term Programs Supervisor

Employment Status Classification: Exempt, Full-Time Salary

Salary Range: $42,120.00 per year

Job Description Summary:

The Housing Resource Community Coordinator is a key member of Community Housing Network's Short-Term Programs team. This position is responsible for working with program participants enrolled in the various department programs, to either obtain or maintain their housing. The coordinator will work with program participants through the assessment process and if eligible also provide housing case management. The coordinator will actively participate in the coordinated entry system as well as the larger service network as needed. The coordinator will also contribute to outcomes and continuous quality improvement at the agency and Continuum of Care level.

*Essential Functions:  *

  • Meet with program participants to complete required paperwork and conduct a full assessment for the applicable grant funding and resources.
  • Provide housing case management services following Critical Time Intervention and Housing First models using a strength-based, person-centered approach.
  • Develop individualized housing plans in collaboration with the participant receiving services.
  • Engage in compassionate and empowering problem-solving conversations with program participants, community partners, and team members.
  • Explore affordable housing opportunities for program participants to assist in identifying housing of their choice that meets program requirements.
  • Conduct monthly home visits with housed program participants to review housing goals, continued eligibility, and exit planning.
  • Document all case notes within the Homeless Management Information System (HMIS) per grant and agency requirements.
  • Explore, identify, and link program participants to community resources related to their household needs.
  • Participate in community-based outreach activities as needed. Some activities may include evening and weekend hours.
  • Continue to refine skills and stay abreast of best practices and program changes through additional training and community events.
  • Navigate participant general and emergency support as needed.
  • Other duties as assigned.

Qualifications and Skills:

  • Experience preferred conducting intakes and working with vulnerable populations (i.e., people experiencing homelessness, domestic violence, at-risk of homelessness, and those who have a mental or physical health disability).
  • Bachelor's degree in social work or human services related field; lived experience, or two years of human service-related experience and/or training, or equivalent combination of education and services.
  • Working knowledge of the Coordinated Entry System and other community resources and services related to housing and supports is essential.
  • Must possess proficient computer skills, and be well organized, proactive, and self-directed. Must also be attentive to details and multitask in a fast-paced environment.
  • Valid driver's license.
  • Personal automobile coverage limits minimum: Bodily Injury $100,000 each person, $300,000 each occurrence Property Damage $100,000 each occurrence.

Travel:

Travel required within the communities served, Oakland and Macomb County.
