Resource Center for Independent Living Veteran Directed Care Person Centered Counselor in Utica, New York
Starting non-exempt salary: $43,992

Job Duties:
The successful candidate will be expected to:
Meet with Veteran to develop a comprehensive person-centered spending plan.
Acknowledges receipt of referral form from VAMC within one business day.
Contacts the Veteran within two business days of receiving referral to schedule an intake home visit.
Conduct quarterly in-person visits to monitor well-being of Veteran and review spending plan.
Communicate effectively with Veteran and/or Authorized Representative and provide counseling if not adhering to spending plan.
Assess, identify, coordinate, and monitor supports and services on an ongoing basis.
Have a valid NYS driver license, have own/reliable transportation, be able to travel, and have a flexible schedule to accommodate Veteran’s need.
Education:
Social Work degree preferred.
Other relevant education in the Human Services field will be considered.
Experience may be considered in lieu of education.
Knowledge, Skills & Abilities:
Must be proficient in Microsoft Office suite applications for preparation of documents, spreadsheets, and data management.
Sensitivity and awareness of issues related to Veterans. Ability to work with diverse population.
Community networking and collaboration, advocacy, willingness to work a flexible schedule.
Excellent customer service skills, strong verbal and written communication and time management skills.
Knowledge of human services, Independent Living philosophy and case management preferred.
Benefits:
37.5 hour work week, 7.5 hour work day, flexible work schedule menu to include the ability to work: a four-day work week, a four-day work week every other week, or a typical five-day work schedule.
