Main Function: Provide person centered services and supports to assist the individuals during Community Habilitation services to achieve independence and maintain their health and safety. Responsibilities include: promoting growth and independence with personal needs and ADLs, monitoring health and safety, building advocacy, decision making and community connections, and completion of required documentation.
Duties and Responsibilities:
- Assist Program Managers with the day to day operations of Community Habilitation services
Assist Program Managers with the oversight of the implementation of participants’ Action plans and safeguards by DSPs
Assist Program Manager with development and completion of Action plans and monthly notes as directed
Assist Program Manager with monitoring the completion of DSP documentation
Assist the Program Manager with scheduling as directed
Assist the Program Manager with staff training as directed
May assume responsibilities of Program Manager during long-term vacancies
- Provision of services for participants
Implement participants’ goals as written in Action plan
Assist/support participants with ADL skills including, but not limited to, bathing, toileting, dressing, dining, mobility, as needed
Teach participants skills to promote independence including, but not limited to: cooking, money management, literacy, socialization and health and safety
Implement behavior guidelines/plans as written
Offer choices and opportunities for participants to engage in and increase leisure skills and community activities and connections
Offer participants choices and encourage them to make healthy choices
Provide support to participants in building friendships and maintaining contact with family and friends
Support participants by providing opportunities to increase decision-making and advocacy skills
Transport and accompany participants to activities of their choice
- Monitor and maintain participants’ safety
Implement safeguards as written in Action plan
Assist with the completion of participant assessments as assigned
Monitor and maintain the safety of participants while in the participant’s home, community, and during transportation
Obtain and maintain CPR/FA certification and follow Agency procedures as need arises
Report participant incidents per Agency standard
Utilize pro-active approach to avert and/or de-escalate crisis situations
Follow procedures to report emergencies or medical/safety needs/changes of participants
- Completes required documentation
Complete all program documentation in a timely manner and per Agency standards. Documentation includes but not limited to: service notes, behavioral data, program outcomes, etc
Completes incident report forms thoroughly and in a timely manner per Agency standards
Completes mileage sheets per Agency standards
- Community and Teamwork
Work collaboratively with participants, families, program staff, co-workers and supervisor
Attend staff meetings and other meetings/committees as directed by supervisor
Attend all required trainings as directed by supervisor and maintain training certifications
Participate in Agency-wide events, advocacy, and/or community education.

Education/Experience:
High school diploma/GED plus one year experience working with people with disabilities required. Basic word processing and e-mail skills are required.
Conditions of Employment:
Must have a valid New York State driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Must be able to lift 75 pounds. Must have an electronic device that supports the Agency’s EVV (electronic visit verification) system and be willing to use the minimal personal data required for EVV. Successful completion of required orientation and annual training. Must be able to demonstrate teamwork and professionalism. Must be able and willing to demonstrate direct care duties, including personal assistance, as need arises. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People with Developmental Disabilities, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the State Central Register of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
