WACOSA Job Coach in Waite Park, Minnesota

All duties to be performed according to the established standard and in compliance with all company policies and procedures while maintaining regular, prompt attendance: Assist in the development of outcomes measurement goals in community work and competitive employment placement. Complete all appropriate paper work and reports required by WACOSA policy, licensure, and county, CARF and other accrediting and funding organizations. Coordinate and participate with internal and external sources to manage each client s placement logistical needs. Provide individualized instruction for tasks to insure client understanding of job expectations. Educate employer and coworkers on client s disability. Responsible for forming a client support team of family, counselor, teacher, employer and coworkers to help client minimize barriers to employment such as transportation, inappropriate behaviors, task behavior, use of break times, communication skills and work schedules. Provide reports and feedback on client s performance and development. Work with employer to ensure that client succeeds with job placement and skills. Function as a member of the Vocational Services team and provide input into the individual planning process. Identify and incorporate different training methods to meet client needs. Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure client protected health information is secure. Travel to multiple community locations for job coaching, meetings and events. Ability to network within the community to create and maintain relationships for vocational evaluations and transition. Maintain working knowledge of all client programs and supervisory needs. Interaction with clients and mentoring of staff that results in: o Independence in all areas of their day o Healthy social interactions o Self-regulated behaviors o Opportunities to use skills in decision making o Work opportunities to use skills in decision making o Opportunities for client Self Advocacy o Continuous use of person-center principles Complete all appropriate paper work and reports required by WACOSA policy, licensure and county, CARF and other accrediting bodies. Maintain positive and ongoing communication as one of WACOSA s primary contact to interdisciplinary team members.
