Basic Job Function Summary

The Hospice Admissions Nurse-RN (Registered Nurse) utilizes critical thinking skills to facilitate comprehensive nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care and regulatory requirements. In addition, the Hospice Admission RN is expected to establish hospice plan of care and communicate according to the guidelines below. Supervision Exercised Provides functional supervision of LPN’s and Hospice Aides.

Minimum Qualifications

• Graduate of an approved Nursing education program

• Current Registered Nurse licensure in Georgia.

• Must have a minimum of one (1) year experience as a professional acute care nurse.

• Home healthcare/hospice experience preferred.

• Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.

• Must have current Driver’s license and reliable transportation to and from work site.

• Initiates and maintains communication with attending physician, Medical Director, IDG, Hospice Manager and/or Supervisor, and/or other agencies as needed to coordinate optimal care and use of resources for the patient/family.
