Job Summary Coordinates the intake of new patients to Home Health. Assists in future patients planning for services. Responds to community requests for information on resources. What You Will Do: Coordinates the intake of new patients to Home Health. Seeks out needed information from appropriate resources to complete the intake process. Collaborates with referral source, physician and patient/family to plan for future agency services. What you will need: High School Diploma or equivalent required. Home Health/Hospice experience preferred. Customer service experience interacting with patients strongly preferred.
