Being a Transition Coordinator has its Advantages!

You will work 4 days at our Mapleton School in Malvern, PA and 1 day at our Audubon School in Norristown, PA.

Every day as a Educational Transition Coordinator you will:

• Support the transition of students in admission to and discharge from school programs

• Assist students with disabilities and their families in obtaining services and skills necessary to transition from high school to post-secondary setting (post-secondary education, employment, or long-term support).

• Facilitate the transitions of Pennsylvania clients in admission to and discharge from center educational programs

• Help coordinate the employment of client workers, collaborate with the interdisciplinary team, attend IEP meetings and maintain communication with school districts and families/guardians on issues related to the transition at discharge.
