The Care Coordinator navigates the health care system on behalf of high need patients, supervises and provides care coordination, plans for education, and provides for patient safety and quality of care. The incumbent will serve as an agile problem solver on multiple levels, practicing professionally, ethically, and promoting patient advocacy. *$5,000 Sign-on bonus for new-to-NCH employees only. Responsibilities Screening and identification of patients (Frequently) Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination (Continuously) Completes comprehensive Health Risk Assessment (Frequently) Develops family-centered plan of care (Frequently) Facilitates communication and collaboration amongst the healthcare team (Continuously) Develops patient?s/family self-management skills through education and resource provision (Continuously) Manages transitions between settings, caregivers and providers (Frequently) Provides monitoring and follow-up across the continuum of care (Continuously) Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services (Occasionally) Participates in orientation and continuing education of staff and students as appropriate (Frequently) Qualifications MIMIMUM REQUIREMENTS: 1. Current Social Work license in Ohio required. 2. Masters Degree in social work, required. 3. LSW, LPC, (LISW or LPCC preferred) 4. Two years experience in social work required 5.
