POSITION: Personal Care Aide (PCA)

General Purpose: A Personal Care Aide (PCA) provides personal care and incidental / instrumental activities of daily living as assigned to patients / clients in the home setting as permitted by the policies and procedures of Community Care Home Health Services (Company) and any other federal, state , local law, or regulation. The Personal Care Aide is under the direct supervision of the licensed RN. The PCA provides care in accordance with the DOH Matrix: Permissible and Non-Permissible Activities: PCA Services. Essential Functions: Assist, perform, and train patient / client and family in activities of daily living such as bathing, grooming, toileting, and adequate nutritional intake. Assist as assigned with ambulation and transfers. Maintain a clean, safe, and healthy patient / client environment. Assists patients / clients as assigned with incidental activities of daily living such as shopping, meal preparation, socialization activities, homemaking, and medication reminders. Utilize basic infection control measures such as hand washing and personal protective equipment (PPE) appropriate to the tasks and assignment. Use proper body mechanics. Engage is respectful social interactions with the patient / client, including friendly conversations and empathic support as well as respecting clients privacy and property. Exhibit positive attitude, behavior, demonstrate respect to other employees, clients, maintain confidentiality of all information pertaining to the patients / clients and their families. Organize self to carry out visits / shifts and tasks. Attend mandatory Company In-Services and provide requested documents to keep employee files current. Follow the assignment sheet / service plan performing only tasks that are assigned. Document and report and noticeable changes in patients / clients physical condition, attitude, reaction, appetite, etc, to the supervisor / manager in a timely manner. Report all client and employee incidents / accidents to your supervisor / manager immediately. Complete other assignments as requested and assigned. May have access to personal health information (PHI) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization / department. Minimum Education & Experience Requirements: Successful completion of a Personal Care Aide (PCA) Training Program approved by NYS Department of Health and NYS Department of Education. Graduate of approved Alternative Competency Demonstration (ACD) program. Able to effectively communicate with all levels of the workforce. Able to hear , speak, write, and read in a manner understood by most people. Able to read and interpret technical instructions related to providing services of the patient / client and use equipment required in providing services. Able to prioritize multiple tasks to effectively deal with multiple changes based on client needs and scheduling. Working Conditions & Physical Effort: Work is normally performed in a residence, or senior living facility. Work may involve frequent exposure to communicable diseases, bloodborne pathogens and/or other potentially infectious or hazardous materials that require following safety precautions and using work practices that require the use of personal protective equipment. Able to travel from assignment to assignment within geographic area served.
