Reference #: 4504835 THE POSITION Luzerne-Wyoming County Area Agency on Aging is seeking a dedicated individual to join our team as a part-time Aging Case Aide 2. In this paraprofessional position, you will be responsible for supporting the Social Services Department staff, primarily in the Care Management Unit, as well as the Intake/Assessment Unit and Waiver Unit. Geographic area covered is Luzerne and Wyoming counties. The employee reports to an Aging Care Management Supervisor 1. DESCRIPTION OF WORK

This is an Aging Case Aide 2 position with the Social Services Department at the Area Agency on Aging. Within this role you will be responsible for maintaining on-going-proficiency in the use of the statewide Department of Aging data base known as Well-Sky or SAMS. You will receive Weekly Service Status sheets from the AAA's contracted providers. Responsibilities include reconciling the service status noted on the status sheets with the consumer's response to the actual service status by: Contacting the consumers by telephone to determine status of service and satisfaction with services within 2 weeks after an initial referral has been made to a contacted service provider. Recording responses to the 2-week follow up telephone calls in a standardized format in the journal sections of the AAA SAMS Data Base. Contacting the assigned Care Manager with any issues or concerns.

Work Schedule and Additional Information: Part-time employment. Work hours are 9:00 am to 2:00 pm, Monday - Friday. Salary:In some cases, the starting salary may be non-negotiable. QUALIFICATIONS Minimum Experience and Training Requirements: One year as an Aging Case Aide 1; or Two years of public contact work, including one year in a human services program; or Any equivalent combination of experience and training.

Hiring preference for this vacancy may be given to candidates who live within Luzerne or Wyoming County.
