Summary

The Behavioral Health Paraprofessional has personal experience and stability in recovery from substance abuse. This person participates in providing direct services to clients such as Peer group sessions and individualized educational sessions, engaging and assisting clients with their recovery as well as accessing community resources and support.

Essential Duties and Responsibilities

Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with the consumer, their family, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the individualized service plan are implemented.

Serve as a role model to clients showing by example that recovery is possible and attainable; share his/her own recovery story as appropriate and work recovery alongside client; teach the value of every individual\'s recovery experience.

Work with clients to establish and achieve specific recovery goals; assist in developing life and empowerment skills and techniques. Assist in obtaining services that suit the individual\'s recovery needs by providing information on community resources and groups that may be useful.

Maintain the client\'s comprehensive clinical record with attention to accuracy and completion, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, compliance reporting, discharge planning). Ensure accurate and timely completion of all required documentation for record-keeping and billing purposes.

Manage weekly appointment calendar, provide, and assist clients with intake packets as scheduled; and review and score screening/assessment tests. Screen and assess all persons on caseload for financial entitlements (AHCCCS, SSI/SSD, etc.)

Prepare materials to facilitate peer support group sessions and provide individual sessions ensuring clients are meeting treatment plan requirements.

Participate in case staffing and conferences or other meetings with or without the person or his/her family participating. Perform case management functions associated with caseload by policy.

Coordinate and consult with colleagues to keep all informed of issues and progress. Attend and participate in staff meetings and trainings.

Essential Duties and Responsibilities

Provide continuous evaluation of the effectiveness of treatment through the ongoing assessment of the person and input from the person and relevant others resulting in modification to the service plan, as necessary.

Pursue best practice outcomes for individuals with mental illness including continuing education, employment, independent housing, and community tenure.

Adhere to/maintain client confidentiality according to HIPAA and Confidentiality of Alcohol and Drug Abuse Patient Records (42 CFR Part 2); Knowledge and compliance with WGA policies and procedures; other duties as assigned.
