Description LSS of WI & Upper MI is currently seeking a Service Coordinator to serve our tenants in the Northern Wisconsin Area. The role is based in Wausau and will require travel across Northern, WI. The schedule will be coordinated to accommodate travel as best as possible, include mileage reimbursement and occasional hotel accommodations. Ashland and Minocqua site visits would be required one time per month, the others ( Tomahawk, Stratford, Rhinelander, Wittenberg, Antigo, Schofield) will have a coordinated schedule either weekly, bi-weekly or monthly. Our program is designed to promote and support self-sufficiency of those individuals residing in properties, facilitating a variety of support services within the residential community. This is a 36 hour and benefit eligible role. Under the direction of the Program Supervisor, the Service Coordinator HUD provides information and referrals to community connections to tenants following guidelines, as defined in the HUD Housing Handbook Section 202, who need supportive community services to maintain self-sufficiency. Essential Duties and Responsibilities: This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities, as management may deem necessary from time to time. Provide general informal case management, including intake, and referral services to all tenants needing such assistance when from case management is not available. Assessment of health, psychological and social needs. Development of an individually tailored case plan for services and periodic reassessment of the tenant\'s situation. Establishes linkages with all agencies and service providers in the community: shops around to determine/develop the best \"deals\" in service pricing to assure individualized, flexible and creative services for the involved tenant(s).
