MADRONE HOSPICE RN Case Manager in Yreka, California
Madrone Hospice is currently looking to hire:
- RN Case Manager -Assess patient/family needs with the patient's attending physician, develop, review and revise the patients plan of care, provide skilled nursing and supportive care in accordance with the attending physician's orders and plan of care, meet with Interdisciplinary Team weekly to reassess plan of care.
- Application Question(s):
- Possess a current license to practice as a registered nurse in the State of California with at least one year of work experience as a professional nurse, preferably in a home health setting.
